Forty is not a magic number in medicine. There is no master checklist to specify what happens to the body at that age. In fact, 35 might be a more significant birthday. It might represent a biological peak, according to Michael Moore, an Orlando internist who celebrates his 40th birthday today. "The progression of your life is more or less like a hill," Moore said. "Thirty-five is historically about the age when things start to go down . . . but it's a very individual and variable thing." After age 35, the body begins a gradual descent into older age, with an emphasis on gradual. That said, Moore named several general changes in our bodies we can expect to feel at - or around - the age of 40.

- The proportion of body fat to muscle increases.

- There is more soreness after physical activity.

- Pulmonary function and aerobic capacity start to decrease. In other words, as Moore puts it, 40-year-olds "get short of breath quicker." While diet and exercise can slow some of the effects of aging on the body, the eyes are a different matter. At the Florida Eye Clinic in Orlando, Jane Mayfield sees a slew of patients, usually ranging in age from 40 to 42, who come in when they can't focus anymore. It's a sign of this age - when the lens of the eye starts to lose some of its elasticity, said Mayfield, an optometric assistant. What that means is that Mayfield orders lots of bifocals and reading glasses for 40-year-olds. While 40-year-olds debate whether they're old or not, the health care community has already decided. "The physical exam of a 40-year-old is different than the physical exam of a 39-year-old," Moore said. "There are different rules." The American Cancer Society, for example, steps up its recommendations for baseline mammograms for women over 40. After 40, mammograms are recommended either annually or every other year. Metlife's guidelines for annual physical exams call for an EKG and a chest X-ray every five years, for patients 40 to 60 years of age (only one of each is recommended for patients 20 to 40 years of age). Tests to detect colon cancer and diabetes are also recommended for over-40 patients. Moore notices that men are often more rattled than women are by many of the physical changes of midlife - possibly because women are more used to regular checkups for everything from Pap smears to mammograms. Both sexes want to slow the more obvious signs of older age: thinning hair and wrinkling skin. Moore gets lots of requests for Re tin-A and Rogaine, a minoxidil-based drug found to stimulate hair growth. Moore, however, doesn't find he is monitoring his age all that closely. On his birthday party invitations, he listed the event as a "Celebration of the 40s," alluding to the era and not his age. "I feel young," said Moore, who is lean and fit. "I feel a lot younger than I think I'm supposed to feel at 40."

In fact, given that IQ tests are relatively recent inventions, there is no way of definitively knowing who has the highest IQ of all time. All of those caveats aside, here are 35 people with IQs (either based on testing or an estimate) higher than Einstein’s.
