Many women expecting twins have a vaginal birth However, you won't be able to give birth vaginally if your twins' placenta or placentas are covering your cervix ( placenta praevia ) near the end of your pregnancy. A caesarean is your only birth option in this case.Otherwise, your type of twin pregnancy and your hospital's policy will influence whether or not you have a vaginal birth. Hospitals especially tend to have their own policies about women who are expecting identical twins who share a placenta (monochorionic).Some hospitals may assess you individually to see whether monochorionicity makes a difficult birth more likely. Other hospitals may recommend caesareans for all monochorionic twins, regardless of whether or not there are complications.Ask your obstetrician and midwife what your hospital's policy is, and discuss what you would like to happen. Ask if the doctors are experienced in helping twins to be born vaginally. Your local hospital should be aware of the national guidelines on multiple births and on caesareans.Your chance of having a vaginal birth may also depend on when you go into labour. Twins that come early are more likely to be born vaginally than those born at term.Your babies' position when you go into labour is another factor. If at least your first twin is in a head-down position, a vaginal birth may be possible.Having twins vaginally is likely to mean that your birth is medically managed, with continuous monitoring . This in turn may increase the chances that you'll need a caesarean.However, the degree of management during labour will depend on how your pregnancy has gone, as well as your hospital's policies and facilities. In early labour, you may be free from monitoring and able to use the birthing pool. Some hospitals have wireless monitoring, so once you need it, you'll at least be able to move around easily.It's best to be open-minded about trying for a vaginal birth. Labour is unpredictable, and sometimes it's safest for you and your babies if you have a c-section.About two thirds of twin caesareans are unplanned. This may be because:Very occasionally, one twin may be born vaginally, but the second is born by c-section, because he's become distressed. This is rare and happens in less than five per cent of twin births.If you've previously had a caesarean section it may be harder for you to weigh up your birth choices. Your obstetrician may advise caution about trying for a vaginal birth after caesarean (VBAC). It's harder to be confident about the safety of a VBAC in a twin birth.It's worth asking your midwife if there are any antenatal classes for women expecting more than one baby in your area. Joining these classes will help to prepare you for parenthood, and is a great way to build a friendship group of other mums and dads of twins after the birth.There may also be a twins club where you can meet other parents-to-be of multiples, or parents who already have twins.
